Nebraska’s Safest Companies and Workplace Driving Safety Award

Welcome. As a new applicant this application may take you 20-30 minutes. You may save your application and return to complete if necessary.

1. Register your user name and email here. This is a User Name of your choosing.  NOTE:   User names can NOT be changed, it will be attached to your company account.  It is recommended that you use something associated with the company and NOT your personal name or email.  You will receive an email that will provide you a link to setup your password and login to the system (check your spam folder if you do not receive the email right away).
2. Set up your password and login via the link sent to you in the email.
3. Begin the application process.
4. You can choose to save an incomplete application by clicking the “Save and Continue Later” link at the bottom of the form. This will provide you with a unique link that you can use to pick up where you left off. This link is valid for 30 days. Once the completed form is submitted, changes cannot be made.
5. If you are not able to complete your application, return to this page and Log In (top right corner) using your previously set up username and password.
6. Once your application is complete and submitted you will receive an email copy of your submission for your records.
7. Complete your application on or before February 28, 2026. All companies will be notified by the end of March regarding the status of your application.

• All organizations must be a member in good standing of the National Safety Council – Nebraska Chapter, or a National Safety Council Member with a Nebraska service package.
• All stats used are for January 1, 2025 thru December 31, 2025.
• Organizations must have a written injury/illness program or equivalent. If you have not received an award previously, please include a copy of your program with your awards submission. Those who have received awards previously need not resubmit unless they have made program changes.
• New applicants must submit a copy of OSHA 300A log summaries or equivalent for 2023, 2024 and 2025. Re-applications submit 2025 summaries, however, be sure all numbers are provided on form.
• All questions must be answered completely, all numbers must be provided in the Injury and Illness Record. Incomplete applications will not be reviewed.
• Applications can be saved online and completed at a later date. Once complete you will receive an email with a copy of your application information.
• When entering numbers please do not use commas. Enter as 1000 not 1,000.
• Employee hours include only those on your company payroll.
• Do not include any non-work time, even though paid, such as vacations, sick leave, holidays, etc.
• Go online to review your rates and determine your NAICS code.
• The awards committee decision will be final.
• Participating companies who do not receive an award may have a helpful “walk-through” inspection by the National Safety Council – Nebraska Chapter consulting staff.
• All award entries and documentation will be held in confidence.
• Please complete this application no later than February 28, 2026.

Safest Company Application

• 2023, 2024, and 2025 OSHA 300A summary or supporting documents if not under OSHA
• Safety policy declaration
• Written injury/illness prevention plan
• Company logo in an EPS format

Workplace Driving Safety Award Application

• Information about your company’s driving programs and policies
• Number of vehicles on the road (to include company owned + personal vehicles used for business.  Personal = any vehicle used where mileage is reimbursed)
• Number of reported driving crashes while on the job including motor vehicle fatalities

Criteria for determining if and when a vehicle crash is counted

Vehicle crash should be counted when meeting the following criteria:

• A bodily injury occurs (anytime) and/or when damage occurs to a vehicle when it hits, or is hit by, another vehicle, or other object.    For example – collision in an intersection, rear-ended by another vehicle, backing into an object, striking an animal while driving or any vehicle incident involving bodily injury, etc.

Vehicle crashes should not be counted when meeting the following criteria:

• Vehicle damage occurs with no bodily injury and results from incidents other than collision.   For example – damage while the vehicle is parked or otherwise unattended, hail or flood damage, damage due to theft or vandalism, etc.

• Company Logo in an EPS format.
