PLEASE read before proceeding and submitting your application!  Some of our process may have changed for the 2024 application.

New 2024 Applicants:
Welcome.  As a new applicant this application may take you 15-30 minutes.  You may save your application and return to complete if necessary.

  1. Register your user name and email here. This is a User Name of your choosing.  NOTE:   User names can NOT be changed, it will be attached to your company account.  It is recommended that you use something associated with the company and not your personal name or email.  You will receive an email that will provide you a link to setup your password and login to the system (check your spam folder if you do not receive the email right away).
  2. Set up your password and login via the link sent to you in the email.
  3. Begin the application process.
  4. You can choose to save an incomplete application by clicking the “Save and Continue Later” link at the bottom of the form. This will provide you with a unique link that you can use to pick up where you left off. This link is valid for 30 days. Once the completed form is submitted, changes cannot be made.
  5. If you are not able to complete your application, return to this page and Log In (top right corner) using your previously set up username and password.
  6. Once your application is complete and submitted you will receive an email copy of your submission for your records.
  7. Complete your application on or before February 29, 2024. All companies will be notified by the end of March regarding the status of your application.

Returning Applicants:
Welcome back.

  1. 2024 applicants can use their User Name and password from last year.  Can’t remember?  Click on the Login link (upper right corner of this page) to retrieve your information.  Still having an issue?  Email: sbooth@safenebraska.org and we will reset your information.  (or create a new account using a different email address, however, please note that if you change your email address your previous year’s information will not load automatically.  You will need to re-enter your information.)
  2. Login using the Login button on the upper right of this page.
  3. Begin the application process.  Please note – some of the questions have changed and therefore your previous answers may/may not be automatically completed.  Please read all questions to ensure that your answers are correct.  
  4. You can choose to save an incomplete application by clicking the “Save and Continue Later” link at the bottom of the form. Nebraska’s Safest Companies Awards Program Information

If you have questions about the program or application, please contact National Safety Council – Nebraska Chapter, sbooth@safenebraska.org.

  • All organizations must be a member in good standing of the National Safety Council – Nebraska Chapter, or a National Safety Council Member with a Nebraska service package.
  • All stats used are for  January 1, 2023 thru December 31, 2023.
  • Organizations must have a written injury/illness program or equivalent. If you have not received an award previously, please include a copy of your program with your awards submission. Those who have received awards previously need not resubmit unless they have made program changes.
  • New applicants must submit a copy of OSHA 300A log summaries or equivalent for 2021, 2022 and 2023. Re-applications submit 2023 summaries, however, be sure all numbers are provided on form.
  • All questions must be answered completely, all numbers must be provided in the Injury and Illness Record. Incomplete applications will not be reviewed.
  • Applications can be saved online and completed at a later date. Once complete you will receive an email with a copy of your application information.
  • When entering numbers please do not use commas. Enter as 1000 not 1,000.
  • Employee hours include only those on your company payroll.
  • Do not include any non-work time, even though paid, such as vacations, sick leave, holidays, etc.
  • Go online to review your rates and determine your NAICS code.
  • The awards committee decision will be final.
  • Participating companies who do not receive an award may have a helpful “walk-through” inspection by the National Safety Council – Nebraska Chapter consulting staff.
  • All award entries and documentation will be held in confidence.
  • Please complete this application no later than February 29, 2024.

Application Checklist

First time applicants must include the following:

  • 2021, 2022, and 2023 OSHA 300A summary or supporting documents if not under OSHA
  • Safety policy declaration
  • Written injury/illness prevention plan
  • Company logo in an EPS format

Re-applicants must include the following:

  • 2023 OSHA 300A Summary (do not include a log with employee names) or supporting documents if not under OSHA
  • Company Logo in an EPS format.

Celebration of Safety Award Luncheon

The Annual Celebration of Safety is scheduled for Wednesday, May 1, 2024 at the LaVista Convention Center, LaVista, Nebraska.   Our special guest speaker for this year will be Lauren Sisler, ESPN broadcaster.
